Following the change in clinical system at the practice to SystmOne, access to online services is now via SystmOnline.
To register you will need to come into the surgery with photo ID. Previous login details for Patient Online (issued before June 2016) will not work for SystmOnline. If you have not previously completed the ID check (in place since September 2015) then please download the information leaflet and registration form (links further down this page).
We have had many patients asking why it was necessary to change their online access login details. The surgery changed clinical operating system from Emis Web to SystmOne as part of the East and North Hertfordshire CCG initiative to have all primary care providers using the same system. Emis Web and SystmOne are separate commercial products and as such the online interfaces for each are not compatible. It is similar to changing bank accounts: your online banking details for your old bank will not enable you to access your new account.
Once registered, you will be able to use your online account to:
- book routine GP appointments
- cancel appointments (including nurse or HCA appointments)
- order repeat prescriptions
- view problem codes and test results
- view current medication, allergies and your NHS number and
- update your contact details.
NB: Not all of the services within SystmOnline are currently available at Church Street Partnership, and this is clearly indicated in the online service request area. We do not currently offer Full Clinical Record. Questionnaires will be made available to patients with some long-term health problems to help manage their condition: we will be trailling the asthma questionnaire from January 2018.

Please note that parents do not have an automatic right to access the records of their children who are aged 13, 14 or 15: the consent of the child must be obtained. Once they reach 16 years of age, the young person must apply for their own online registration details.
Increased levels of access
Following changes to the GP contract, practices across the country are required to make more of a patients' medical record available to that patient online, if they wish to access the information. Due to these changes the process for registering for online services has become more stringent. Each patient wishing to register for online services must first read the information leaflet, also available from the surgery, and complete a registration form. You must come into the surgery, with photo ID and proof of address to complete the registration process. Acceptable forms of ID include a passport or EU driving licence.
Each person must register for themselves. Parents (or those with parental responsibilty) can register children under 13 years of age for online services. We will need to see the photo ID of the parent and the birth certificate for each child. Once the child reaches 13 year of age, access to online services will be suspended. It can be reactivated if the child gives their consent to their parent having access. This must be done in person, with ID.
At 16 years of age online access accounts will be suspended and only the patient may re-register.
We realise that these changes may inconvenience some patients. We are following advice from the Royal College of General Practitioners and we are certain that our patients will appreciate that we take confidentialty extremely seriously.

We hope that patients will find the online services useful. However, if an individual abuses their online account we will block the access and that patient will need to use the other means of booking appointments, requesting repeat prescriptions etc.
Inappropriate use includes, but is not restricted to:
- sending abusive messages
- booking, then wasting appointments
- over ordering medication
- repeatedly requesting services that are not available.
