Join Patient Participation Group

Church Street Partnership has an established Patient Participation Group (PPG) to gain the views of our registered patients and ensure they are involved in decisions about the range and quality of the services provided by our practice.

If you are interested in becoming a PPG member, please submit your details using the form below. Providing this information will help us to ensure that the PPG is as representative as possible of our practice population as a whole. The information you supply will be used lawfully, in accordance with the Data Protection Act 1998.

NB: This is not the form to use if you wish to register as a patient at the Practice.

The contact information you have provided to join the Patient Participation Group will be shared with members of the PPG, such as your name and email address. This is for the purposes of planning meetings and sharing information relating to the patient group. You may leave the group at any time and your information will no longer be shared with the group.

The collection and use of the information you supply on this application form will be used for the sole purposes of demonstrating group demographics for membership and the means of contact for group participation. We rely on the lawful basis of consent to collect and process both personal and special category data outlined on this application form. Only anonymised data that cannot identify you will be used for purposes beyond this.
